STEP 3: Prioritisation
When you submit an incident report the incident will be fresh in your mind. Based on what you enter about the incident in terms of degree of impact, level of care and treatment required for the person affected, an objective measure of harm is automatically calculated. Harm is represented as an incident severity rating (ISR) - from ISR 1 (most severe) to ISR 4 (near miss).

Degree of impact
Meaning what?
What level of harm did the person affected experience as a result of the incident.
Level of care
Meaning what?
What level of care was required within the health system as a result of the incident for the person affected?
Treatment required
Meaning what?
What type of treatment was given to the person affected as a result of the incident?
Organisational severity
Meaning what?
If it's a non-clinical, non-staff incident, the level of impact on the organisation is rated. Organisational ISR is based on things like media impact, interruption of services, and property damage.
