An event or circumstance...
Meaning what?
It might happen suddenly or it may have been going on for a while undetected.

...that lead or could have lead...
Meaning what?
It may be something that happened or something that almost happened. If someone or something intervened to stop the incident occurring, that counts as an incident and it should be reported.
Eye button 3 of 3
...to unintended or unnecessary harm.
Meaning what?
The incident resulted in a person experiencing harm that was not part of the delivery of care.
