Step 7: Feedback
Feedback is the final component of incident management. For ISR 1 & 2 clinical incidents, principles of open disclosure advocate that feedback should be provided to the patient or the family/carer. But what about staff?

Staff involved in an incident need to be informed by their manager of the outcome arising from an investigation.
For ISR 1 incidents, the findings of summary RCA reports are to be provided to the relevant clinical teams.
Regular reports on trended aggregate incident data, outcomes of in depth case reviews and other forms of investigation are to be provided to clinical teams, ward staff, and management teams.
Feedback is to include information on the final outcome of incident investigations and improvements achieved as a result of these reviews.
This seven step system began with a member of staff identifying that an incident or a near miss event had occurred. It ends with staff receiving feedback about what has been done to prevent the same thing from happening again.
